Community Support Worker


Referrals to the community support worker are targeted at patients who need some extra support to help remain independent, who do not yet meet the threshold for a social care package. Examples of support include taking patient’s shopping, finding and attending social groups with the patient to help prevent social isolation. If you feel yourself or a family member or friend would benefit from a referral to our community support worker, please contact reception who will arrange the appropriate steps.


Patients can also call the community support worker themselves on: 0114 2057120 from 8.45am to 5.15pm Monday to Friday.
